Requesting access happens where you sign in

The access-request form now lives on the sign-in card in the console, as the other half of one question: you are either signing in or asking to. Every Request access button leads there. The form used to have a second copy on the home page, and it was failing in the quietest possible way — your request was recorded, and the page did not move, so there was nothing on screen to tell you it had been sent. If you asked for access and never heard anything, the request is with us; nothing was lost.
